Certain services may only be available in states where Clean Rx LLC and its affiliated clinical partners are authorized to operate. Availability may change from time to time without prior notice.

To use the services, you must be at least 18 years old, located in a jurisdiction where the services are offered, and legally capable of agreeing to these Terms and Conditions.

Meeting those baseline requirements does not guarantee that you will receive treatment, a prescription, or any specific service. Clinical eligibility is determined by a licensed provider.

You are responsible for obtaining compatible devices, internet access, and any software needed to use the Clean Health Rx websites, hosted intake flows, and portal tools. Carrier, messaging, or data charges may apply.

Telehealth may involve diagnosis, treatment, follow-up care, patient education, prescription review, and exchange of medical information using audio, video, secure messaging, or other electronic means when you and a provider are not in the same physical location.

Electronic systems used in connection with the services are intended to use commercially reasonable privacy and security safeguards. Even so, no internet-based system can be guaranteed to be completely secure or uninterrupted.

Clean Rx LLC understands the importance of privacy and confidentiality. Our use of personal information is governed by our privacy practices, applicable law, and the privacy obligations of the affiliated clinical organizations and service providers involved in your care flow.

Use of the intake and portal experience may also involve Bask Health infrastructure and related platform terms or privacy practices where applicable.

Clean Rx LLC provides a technology, brand, intake, and patient-flow experience. Clean Rx LLC is not itself a medical group and does not directly provide medical advice, diagnosis, or treatment through the site.

Medical services, if any, are provided by independent licensed clinicians or affiliated professional entities. By using the services, you understand that any provider-patient relationship is with the treating clinical entity or provider, not with Clean Rx LLC.

By providing your phone number or email address, you consent to receiving transactional communications related to intake, appointments, orders, shipping, care coordination, and account activity. If you separately opt in, you may also receive marketing communications.

Text messages and email may not always be encrypted. If you choose to send health or other sensitive information using those channels, you do so at your own risk. You may opt out of marketing texts at any time by following the instructions in the message.

A provider using telehealth tools may not have the same information available as they would during an in-person examination. Because of that, recommendations may be limited, provisional, or subject to follow-up testing, further review, or an in-person evaluation.

Some medications or services may not be available through telehealth in all states or situations. By using the services, you acknowledge those limitations.

Except for specific communications from a licensed provider or affiliated clinical organization, content on the website is provided for general informational purposes only and should not be treated as medical advice or a substitute for in-person care.

If you create an account or submit intake information, you agree to provide accurate, current, and complete information and to update it when necessary.
